Admissions

Clients are accepted into the program by way of state-referral, social worker/guidance counselor referral, or walk-in inquiries. All potential clients are assessed to meet program qualifications. Potential clients can be denied into any program due to limited space.

County Placements:
• Girls aged 12 to 23 who are dependents of the courts are referred by county social service agencies and must have proof of pregnancy or may already be parenting a child.

Private Placements:
• Pregnant girls aged 12 to 23 from homeless families or families in crisis must have proof of pregnancy and have the permission of her parent or guardian to resign at one of Lean on Us unity living quarters.
• Family members must be willing to participate in counseling, take the pregnant teen to doctor and hospital visits when possible, provide medical insurance if available, and provide clothing and spending money.
• Fees are requested on a sliding scale to help pay for the board and care of the teen mother. Thanks to our donors, however, no girl in need will be turned down due to the inability of her family to pay for her care.

Outbound
If you would like to make a referral to Lean On Us Unity, please contact the Intake manager at 703-649-4519 and we will return your call within one business day.

Complete an Outbound Application and gather all supporting documents such as proof of age and proof of pregnancy or living child. The application may be completed on line or faxed in with the supporting documents to 301-248-3151.

Your outbound packet will be reviewed and you will receive a response regarding appropriateness for entering the program. If the applicant meets the criteria, a pre-placement interview will be scheduled.

Following the interview a decision for acceptance will be made within 7 days and you will be notified at that time. If you are accepted, a start date and location will be given at that time.

Inbound

The Intake manager will need these additional documents before the placement is finalized:
• Copy of Insurance Card
• Immunization Record
• Copy of Birth Certificate
• Copy of Social Security Card
• Most recent school transcript
• Current IEP
• Educational Evaluations
• Educational Records
• Physical Examination (date of last visit, Dr. name & phone #)
• Dental Examination (date of last visit, Dr. name & phone #)
• Copy of custody order, if the child is not in the custody of biological parents.
• Placement agreement signed by funding agency, placing agency and guardian
